Syndicate Legal Advisory Services prepares the client’s official court forms to ask for a change to an existing family court order.
What’s included:
- Jurisdiction check. Using our state table, we give the client the state’s own published rules on which court may modify an order and the forms it uses.
- Order and case record. The client supplies the file-stamped order the client wants changed, the case number, the court, and the county.
- Intake and information gathering. A structured questionnaire records the client’s own statements: both parties’ names and addresses, the children’s names and birthdates, the date of the current order, the changes the client says have occurred, and the date each occurred.
- Request for order preparation. From the client’s statements, we type the court’s request form and the attachments the client selects, stating the order the client wants changed and the change the client requests, exactly as the client states them. We do not suggest, recommend, or correct requests.
- Client declaration (client’s words). We prepare the client's statement of the facts supporting the request.
- Financial forms (for support changes). We type the client’s own figures on the Income and Expense Declaration and any related worksheet. We do not calculate support, verify figures, or advise on income or deductions.
- Children’s forms (for custody changes). We prepare the client’s selections on the parenting plan and custody attachments, and the UCCJEA declaration if required.
- Stipulation forms (if the parties agree). If both parents agree to the change, we prepare the stipulation and proposed order from the terms the client reports are agreed, exactly as stated. Both parties review and sign.
- Responsive paperwork (if the client is the responding party). We prepare the client’s answers on the court’s responsive declaration and supporting financial forms.
- Service information. We provide a neutral checklist of the court’s published rules on who may serve papers and how, the notice period, and the proof of service forms. The client may not serve the papers personally, and we do not serve.
- Document organizer. We organize the client’s own documents (the order, pay stubs, tax returns, bank statements, medical or school records the client chooses, communications, and prior filings) into a labeled file.
- Fee waiver forms (optional). If the client requests, we type the client’s own answers on the court’s fee waiver forms.
- Filing packet. We provide a printed or digital packet with the prepared forms, copy and signature instructions, and the court’s published filing procedures. The client signs and files, and we do not file unless counsel confirms it is permitted.
- After-filing checklist. A neutral list, from the court’s public guidance, of the steps that follow: a hearing date, service, any mediation or evaluation, the response, and the order after hearing.
- Review and revisions. We make revisions you request to the typed forms before signing, within the review rounds in your package.
