Syndicate Legal Advisory Services prepares the client’s claim form, and, if the client asks, organizes the client’s documents for an internal appeal or a complaint to EBSA.

What’s included:

  • Intake and information gathering. A structured questionnaire records the client’s own statements: the plan name, the plan administrator and claims address (as printed in the client’s own documents), the type of benefit, the claim number, the denial date, and the contact information.
  • Plan claim form typing. We prepare the client’s answers on the plan’s own claim form or appeal form, from the client’s statements and the client’s own documents, verbatim. The client reviews and signs.
  • Client statement. For a written appeal, the client states in the client’s own words why the client disagrees with the denial.
  • Document organization. We organize the client’s own documents (denial letter, plan documents, summary plan description, claim file, medical records the client supplies, pay and employment records) into a labeled file.
  • EBSA information request (optional). If the client asks, we prepare the client’s request for plan documents (which a plan must provide on written request) or the client’s own complaint to EBSA, typed from the client’s statements. The client signs and sends it.
  • Filing and delivery packet. We provide instructions and a checklist for the client to deliver the appeal by a method that gives proof of delivery. The client signs, sends, and keeps copies.
  • Review and revisions. We make revisions you request before the client signs, within the review rounds in your package.
