Syndicate Legal Advisory Services prepares court forms, letters, and organizes records for a grandparent or other relative seeking contact with or care of a grandchild on the forms required in the state and county where the case is or will be filed.

What’s included:

  • Intake and information gathering. A structured questionnaire and document checklist collect the grandparent’s information, the child’s name and age, the parents’ names and status (living, deceased, married, divorced, separated, incarcerated), the child’s residence history for the last five years, the grandparent’s relationship with the child (including time spent together and any time the child lived in the grandparent’s home), any existing custody, guardianship, dependency, or adoption case, any restraining orders, and what the client is asking for.
  • Standing information sheet. We provide the state’s own published description of who may petition, and the client records their own answer to each item.
  • Visitation petition or request. We complete the state’s form petition or request for grandparent visitation, and, where a family case already exists, the request for order form, together with any required cover sheet, UCCJEA declaration (child’s residence history), and local forms.
  • Custody or guardianship forms (optional, additional fee required). Where the client selects custody or guardianship, we complete the court’s form petition and the related forms, such as the consent or notice documents, from the client’s information, following our Child Custody and Conservatorship descriptions where applicable.
  • Declaration in the client’s words. We prepare the grandparent’s account of the relationship with the child and the reasons for the request, in specific facts (who, what, when, where).
  • Relationship records organizer. We organize the client’s photos, messages, calendars, cards, school and medical records the client lawfully holds, and a timeline of contact into an exhibit index.
  • Mediation and counseling forms. Where the state or court requires or offers mediation, we complete the intake or statement form from the client’s information and provide a checklist of what to bring.
  • Notice and service documents. We prepare the notice to the parents and other required parties and the proof of service, from names and addresses supplied. The client cannot serve the papers, so service is arranged through an adult who is not a party, or a registered process server at the client’s cost.
  • Responding to a petition. If the client is a parent or other party served with a grandparent petition, we do not prepare it, since we serve only the grandparent on a given matter.
  • Letters (optional). Over the client’s signature, from facts supplied, we prepare a respectful letter to a parent requesting contact, a request for mediation, or a request for school or medical information the client is entitled to receive. We do not contact the parents or the child.
  • Fee waiver request. If the client cannot afford the filing fee, we complete the court’s fee waiver request from the information provided.
  • Filing logistics. We provide a filing packet with copy and filing instructions, and at the client’s direction handle e-filing or courier filing where the court allows.
  • Hearing packet. We prepare an exhibit index and a checklist of what to bring.
  • Post-order documents. From the order the court actually makes, we prepare a visitation schedule summary and a request for certified copies.
  • Review and revisions. We make revisions you request to the drafts before signing or filing, within the number of review rounds included in your package.
