Syndicate Legal Advisory Services prepares the official court forms to ask a court to establish or declare a parent-child relationship.
What’s included:
- Jurisdiction table check. Using our state table, we give the client the state’s own published rules on which court hears parentage cases, residency and venue requirements, the child’s home state, and the forms used.
- Intake and information gathering. A structured questionnaire records the client’s own statements: the client’s name and role (mother, alleged father, presumed parent, or other), the other parent’s name and last known address, the child’s name, birth date, and place of birth, the dates the parents lived together or were married, whether the child’s birth certificate names a parent, and whether any declaration, order, or agency case already exists.
- Petition preparation. From the client’s statements, we prepare the court’s petition to establish parental relationship (for example, California FL-200) and the related forms, including the client’s selections of requested orders (parentage, custody, parenting time, child support, name, and health coverage).
- Children’s forms. We prepare the UCCJEA declaration (for example, FL-105) and, for any custody or support request, the related forms as the client directs.
- Financial forms (if support is requested). We prepare the client’s own figures on the Income and Expense Declaration and related worksheets.
- Genetic testing information. We give the client the court’s and the state’s published information on how testing is requested and ordered.
- Stipulation and judgment forms (if the parents agree). If both parents agree that one is the child’s parent, and agree on any terms, we type the stipulation and proposed judgment (for example, California FL-235) from the terms the client reports are agreed, exactly as stated. Both parents review and sign.
- Responsive paperwork (if the client is the responding party). We prepare the client’s answers on the court’s response form and supporting financial forms, from the client’s statements.
- Service information. We provide a neutral checklist of the court’s published rules on who may serve papers and how, and the proof of service forms. The client may not serve the papers personally, and we do not serve.
- Document organization. We organize the client’s own documents (birth certificate, hospital paperwork, any declaration, marriage or divorce records, photographs, messages the client chooses to include, proof of payments, and prior orders) into a labeled file.
- Fee waiver forms (optional). If the client requests, we type the client’s own answers on the court’s fee waiver forms.
- Filing packet. We provide a printed or digital packet with the prepared forms, copy and signature instructions, and the court’s published filing procedures. The client signs and files, and we do not file for the client unless counsel confirms it is permitted.
- After-filing checklist. A neutral list, from the court’s public guidance, of the steps that follow: service, the response period, any genetic testing order, a hearing, and the judgment, and of how the client may update the birth certificate through the vital records office after a judgment.
- Review and revisions. We make revisions you request to the typed forms before signing, within the review rounds in your package.
