Syndicate Legal Advisory Services prepares the client’s official court forms to ask for a change of name.
What’s included:
- Intake and information gathering. A structured questionnaire records the client’s own statements: the current legal name as it appears on the birth certificate or other identity document, the requested new name, other names used, date and place of birth, residence and the county where the client will file, and the client’s own statement of the reason.
- Petition preparation. From the client’s statements, we prepare the court’s petition and attachments (for example, California NC-100 and NC-110), including the client’s stated reason and the requested name, exactly as the client states them.
- Minor’s name change (if selected). We prepare the court’s forms for a minor from the petitioner’s statements, including the information about both parents, any consent, and any notice to a parent who has not signed.
- Criminal history and registration disclosures (if the state requires). We prepare the client’s own answers on any state form or declaration requiring disclosure of criminal convictions, registration, or pending cases, exactly as the client states them.
- Gender marker forms (if selected). If the client selects a gender marker change, we prepare the court’s or agency’s form from the client’s statements. Where a provider statement or physician declaration is required, the provider completes it.
- Confidentiality or sealing request (optional). If the client requests it and the state allows it, we type the court’s own form to seal or protect the petition or the order.
- Order to show cause, notice, and publication information. We prepare the court’s order to show cause or notice form where used. We give the client the court’s published rules on newspaper publication, how to choose a qualifying newspaper, and the proof of publication form. The client arranges and pays for publication, and we do not do so.
- Document organization. We organize the client’s documents (birth certificate, marriage or divorce records, driver’s license or ID, passport, Social Security card, court orders, and letters supporting the request) into a labeled file.
- Fee waiver forms (optional). If the client requests, we type the client’s own answers on the court’s fee waiver forms.
- Filing packet. We provide a printed or digital packet with the prepared forms, copy and signature instructions, the court’s published filing procedures, and any fingerprint or background check instructions. The client signs and files, and we do not file for the client unless counsel confirms it is permitted.
- Hearing and decree forms. We type the proposed decree, and any hearing-related forms, from the client’s statements.
- After-order checklist. A neutral list, drawn from public agency guidance, of places the client may choose to update (the Social Security Administration, the motor vehicle agency, the passport office, banks, employers, schools, voter registration, and the birth certificate), with the agencies’ published addresses and websites.
- Review and revisions. We make revisions you request to the typed forms before signing, within the review rounds in your package.
