Syndicate Legal Advisory Services prepares letters, forms, and organizes records for a licensed or approved foster parent (resource parent) on the forms the state agency and court require.

What’s included:

  • Intake and information gathering. A structured questionnaire collects the foster parent’s information and license or approval details, the child’s information (only what a form requires), the placement date, the caseworker and agency, the court and case number (if provided), notices received, and the issue the client is facing. We do not collect confidential details beyond what the forms require.
  • Document organizing and review. We organize the client’s own placement records, notices, caseworker correspondence, medical and school records for the child (as lawfully held by the foster parent), photos, and a daily log into a timeline and index.
  • Caregiver information form for the court. We complete the state’s caregiver information form (for example, the Caregiver Information Form, JV-290 in California) from the foster parent’s own account of the child’s health, education, behavior, and the placement, for the foster parent to sign and submit to the court or the social worker.
  • Request for notice and to be heard. From facts supplied, we prepare a written request to the agency or the court clerk that the foster parent receive notice of hearings and reviews, and, where the state provides a form, a request to be heard or to submit information at a hearing.
  • Placement change and removal notice forms. Where the agency proposes to move a child, we prepare, from the foster parent’s statements, any written objection, request for review, or request for a hearing the state’s form allows, and a request for a copy of the written notice and the reasons.
  • Agency grievance and administrative review forms. From facts the client supplies, we complete the agency’s grievance, appeal, or fair hearing request regarding a licensing action, a corrective action plan, a denial, a rate or payment decision, or a decision about a child’s placement, using the agency’s form.
  • Reimbursement and payment forms. From records the client supplies, we complete the foster care payment, special care rate, clothing allowance, or reimbursement request forms the state provides, and a request for a payment history.
  • Medical, education, and records forms. We prepare requests to the agency for the child’s records the foster parent is entitled to receive, a request for health or education consent authority paperwork, and school enrollment documents, from facts supplied and on the agency’s forms. We do not obtain records from third parties and do not share the child’s confidential information beyond what the forms call for.
  • Licensing and approval renewal paperwork. From information supplied, we complete renewal, update, and change-of-household or change-of-address forms for the foster home license or approval, and organize required training and background check documents.
  • Complaint forms. From facts the client supplies, we complete a complaint to the state’s child welfare agency, the foster care ombudsman, the licensing agency, or the state attorney general, in the foster parent’s words.
  • Visitation and sibling contact requests. From the foster parent’s statements, we prepare a request to the agency or court regarding contact with the child’s siblings, former caregivers, or, after a placement ends, a request for contact information or a final visit, using the state form.
  • Request to change court order (limited). Where the state allows a foster parent to ask the court to change an order, we complete the court’s form (for example, the Request to Change Court Order, JV-180 in California).
  • Adoption and guardianship interest forms (optional - see "Adoption" services). Where the foster parent wants to be considered as a permanent placement, we prepare the agency’s letter of interest or intent to adopt or become a guardian, from the client’s decision, and, if the court has set an adoption or guardianship process, we follow our Foster Parent Adoption. Foster Parent Adoption is not available with this package.
  • Service documents and filing logistics. We prepare proof of service for papers the client files or sends, and a filing packet with signature, copy, and delivery instructions. At the client’s direction we handle e-filing or courier filing where the court or agency allows.
  • Review and revisions. We make revisions you request to the drafts before sending or filing, within the number of review rounds included in your package.
