Syndicate Legal Advisory Services prepares the court forms and supporting documents for an adoption in which the child is placed through a licensed adoption agency, on the forms required in the state and county where the case is or will be filed. The agency, not Syndicate Legal Advisory Services, performs the home study, placement, supervision, and required reports.
What’s included:
- Intake and information gathering. A structured questionnaire collects the adoptive parent(s)’ information, the child’s information, the agency’s name and case contact, and the placement date and history.
- Adoption petition package. We prepare the Adoption Request, the Adoption Agreement, the proposed Adoption Order, and the required declarations and cover sheets.
- Agency-related paperwork. We organize and prepare the documents that accompany the petition, including cover letters and transmittals to the agency, and incorporate the agency’s placement documents, relinquishment documentation, and reports into the filing packet as provided by the agency.
- Placement and consent documentation. We prepare client-side acknowledgments and any supplemental declarations the court requires, and assemble the agency-provided relinquishment or consent paperwork for filing.
- ICWA documents. We prepare the Indian Child Welfare Act inquiry and notice-related forms based on the information you provide.
- Background and disclosure forms. We prepare the adoptive parent disclosure forms and related court-required attachments.
- Hearing preparation. We prepare the hearing packet, including proposed orders and a checklist of items to bring.
- Filing logistics. We provide a filing packet with signature, notarization, and copy instructions. At your direction, we can handle e-filing, courier filing, and proof of service preparation.
- Post-hearing documents. We prepare the final Adoption Order, a request for certified copies, and a checklist for the amended birth certificate application.
